The problem
A busy psychiatric provider sees 10 to 25 nursing home patients a day. After every visit, he typed the note into his EHR: about 20 forms and 150 fields per new patient, with answers that open more questions. It took 4 to 8 hours a day, and his ChatGPT copy and paste routine always missed something.
What I built
An AI-powered system on his own computer:

- 2AI reads each form and fills it at a calm human pace
- 3It saves drafts and never signs. He reviews and signs
- 4He changes the rules himself in plain English
The hard part
The first version mapped every field by hand. It hit 90% on the test patient, then fell below 60% on real patients because the forms changed from patient to patient. So the system was rebuilt: now AI reads every form fresh each time and decides what goes where.
The results
- 1 to 4 minutes per patient, several at once
- 99.9% accuracy
- A full day of notes in about 20 minutes, not 4 to 8 hours
- He added his second clinical app to the same system
